How to Apply
1. Submit a Letter of Request addressed to the PCSO Chairman/General Manager with a Comprehensive Project Proposal stating the following information:
2. Copy of SEC Registration for NGOs
- Organizational profile
- Name of project, if applicable
- Schedule of activity
- Rationale to include a statement on the availability of resources or lack of access to health service
- Objectives of the activity
- Target number of recipients
- Contact person / contact number/s
3. For financial assistance, canvass/quotation of medicines from the supplier.
