PCSO Medical Assistance Program
Criteria
The following scenarios are eligible to apply for PCSO medical assistance:
- confined in any health facility
- receiving any health care management as an outpatient
- seeking treatment in foreign countries provided that no health facility within the Philippines is capable of delivering a particular procedure.
Coverage
Eligible patients who are currently admitted or discharged with a promissory note duly acknowledged by the health facility or medical center.
Dialysis
Eligible patients undergoing, but not limited to, Hemodialysis and Peritoneal (Continuous Ambulatory Peritoneal Dialysis or CAPD).
Eligible patients undergoing, but not limited to, Hemodialysis and Peritoneal (Continuous Ambulatory Peritoneal Dialysis or CAPD).
Cancer Treatment
Cancer patients who are undergoing medical treatment, such as, but not limited to:

Non-invasive and Minimally Invasive Procedures
- Extracorporeal Shock Wave Lithotripsy (ESWL)
- Endoscopic procedures

- Diagnostic procedures for Coronary Angiogram or Cardiac Catheterization

Documentary Requirements
The following should be accomplished and submitted:
- Duly accomplished PCSO IMAP Application Form.
- Original or Certified True Copy of the updated Clinical Abstract or Medical Abstract signed by the attending physician with license number.
- Any valid ID (patient and representative)
- Authorization Letter from the patient in cases where there is no immediate relative available for interview.
Specific Requirements
Confinement
- Original copy of the Final Statement of Account/ Latest Hospital bill with printed name duly signed by the Billing officer/Credit Supervisor with PhilHealth, Senior Citizen, HMO, MSS/ Discounts deductions
- If Discharged: Validly-executed Promissory Note duly signed by the hospital representative or Certification with remaining balance from the hospital
- For Medico-Legal Cases: Copy of the Vehicular/Police Report
Chemotherapy
- An original prescription with printed full name, signature, and license number of oncologist/ attending physician
- Original copy of treatment protocol with signature, name, and license number of oncologist/attending physician
- Photocopy of Surgical/Histopathology/Biopsy Result
- Index Card for patients with previous assistance
Dialysis (Hemo/PD/Erythropoietin)
- Acceptance Letter from the Dialysis Center/ Hospital accepting PCSO guarantee letter
- Official Quotation from the Dialysis Center/ Hospital (for dialysis)
- A prescription with printed full name, signature, and license # of the attending physician (for Erythropoietin)
- Photocopy of relevant laboratory result/s within the last three (3) months
- Index Card for patients with previous assistance
- For PhilHealth Members:
Medical expense (Hemophilia and Post-transplant medicines)
- A prescription with the printed full name, signature, and license number of the attending physician
- Photocopy of relevant laboratory result/s within the last three (3) months
- Index Card for patients with previous assistance
PCSO Medical Assistance Process
Here are the steps on how you can apply for the Individual Medical Assistance Program or IMAP Program:
- Obtain a Clinical abstract from the patient's doctor. You will also need to present the official hospital billing statement from the hospital's Accounting/Finance Department.
- You may also need to ask for a certification from your barangay captain, certifying that you are a patient's relative and that your family has no means of paying for the hospital bills.
- Make a letter addressed to the PCSO General Manager, Regional Manager, or Chairman of PCSO, stating the patient's condition and the help you need.
- Complete a PCSO IMAP Application Form. The form can be downloaded online through PCSO's website; a link is provided under the 'Documents to Use' section. Or you can also get a copy of the application form at the PCSO Lung Center of the Philippines Satellite Office, PCSO Branch Offices, and ASAP Partner Hospitals.
- You will also need to submit specific requirements depending on the patient's case.
- Make sure you have all the required documents before going to a PCSO center. Also, do not forget to bring a valid ID with you (Patient and Representative).
- It might be a good idea to go to a PCSO center very early in the morning to avoid waiting so long. There are a lot of people also applying for the program.
- The PCSO guarantee letter will be given to partner hospitals, health facilities, or service providers. This is how they provide medical assistance to patients who need it. The guarantee letter assures the hospital that it will be PCSO who will be paying for the patient's medical bill.
Due to the Covid-19 pandemic, PCSO suspended its operations in its main and extension offices. Applications for medical assistance in Metro Manila can already be made online.
The PCSO Medical Access Program Online Application intends to secure financial assistance within the premise of augmenting the costly medical expenses. It operates on a first-come, first-serve basis.
