The Philippine Charity Sweepstakes Office (PCSO) Calamity Assistance Program refers to the provision of assistance to families and individuals affected by calamities/disasters in appropriate form through the Local Government Units (LGUs). It is directly provided to individuals affected by the calamities, health facilities, and authorized agencies and entities created to address the needs of the country and its communities during calamities/ disasters/medical emergencies.
Documentary Requirements:
The PCSO Calamity Assistance Program requires the following documents for:
LGUs:
- Sangguniang Panlalawigan/Bayan/Barangay Resolution authorizing the Local Chief Executive to sign the Deed of Undertaking and to receive the assistance from PCSO
- Photocopy of the government-issued ID of the requesting party
- Data obtained from any of the following: National Disaster Risk Reduction and Management Council (NDRRMC), Provincial Disaster Risk Reduction Management Council (PDRRMC), Department of Social Welfare and Development, Disaster Response Operations Monitoring and Information Center (DSWD – DROMIC)
Health Facilities:
- DOH Situational Report on Emerging Diseases/Cases
- Relevant documents to be submitted at a later date.

- DOH License to Operate
